The Morecambe Bay Investigation was established by the previous Secretary of State for Health to examine concerns raised by the occurrence of serious incidents in maternity services provided by UHMBT, including the deaths of mothers and babies, in the period between January 2004 and 2013.

The Kirkup report was written following an independent investigation into the management, delivery and outcomes of care provided by the maternity and neonatal services at the University Hospitals of Morecambe Bay NHS Foundation Trust from 2004 to 2013. The A-EQUIP model was developed in response to changes in legislation and shaped by the contribution of users of maternity services, midwives, educators, managers and nurses. The lessons learned from the pilot sites have also had a significant impact on the final design of the model.
