Lung-RADS® is a quality assurance tool designed to standardize lung cancer screening CT reporting and management recommendations, reduce confusion in lung cancer screening CT interpretations, and facilitate outcome monitoring.

What does lung-RADS 4A mean?
Risk of cancer 1-2%. Lung-RADS 4A: Suspicious, 5-15% risk of cancer (e.g. new solid nodule 6 mm to <8 mm), repeat chest CT in 3 months or get PET/CT if solid component is ≥ 8 mm. Lung-RADS 4B: More suspicious, >15% risk of cancer (e.g. new or growing solid nodule ≥ 8 mm).
What does lung-RADS category 4X mean?
Lung-RADS introduced a category 4X for lesions that demonstrate “additional features or imaging findings that increase the suspicion of malignancy.” According to Lung-RADS , upgrading a nodule to category 4X is possible only for category 3, 4A, or 4B nodules.

What is a LI-RADS 5 lesion?
LI-RADS 5 indicates a lesion for which the radiologist has 100% certainty that it is diagnostic of HCC. Thus, the lesion should be treated as HCC without a biopsy.
When do you use LI-RADS?
Doctors only use LI-RADS for patients who have cirrhosis or who are at high risk of developing HCC. (No LI-RADS categories will appear if the patient is not at increased risk for HCC or is under the age of 18.)
