Form I-693 reports results of a medical examination to U.S. Citizenship and Immigration Services (USCIS). USCIS requires the examination to establish that applicants who are seeking immigration benefits are not inadmissible to the United States on public health grounds.
When to submit Form I693?
According to USCIS guidance, Form I-693 is valid only when a civil surgeon signs it no more than 60 days before the date an applicant files the application for the underlying immigration benefit; and USCIS adjudicates the application within two years from the date of the civil surgeon’s signature.

For children under 14 years old: Parents or legal guardians may sign Form I-693 for the applicant. For children 14 years and over: The applicant must sign Form I-693.
