How do you initiate Coumadin?

How do you initiate Coumadin?

During warfarin initiation for VTE treatment we suggest the following:

  1. Initiate warfarin as soon as possible following diagnosis of VTE, preferably on the same day, in combination with UFH, LMWH or fondaparinux.
  2. The initial dose of warfarin should be 5 or 10 mg for most patients.

Can you start warfarin outpatient?

Warfarin (Coumadin) therapy should be initiated using validated 5-mg and 10-mg nomograms. Outpatient LMWH is as safe and effective as inpatient unfractionated heparin for treatment of venous thromboembolism in most patients.

When should I start taking warfarin?

Warfarin can be started as soon as the diagnosis is confirmed. Heparin should be continued until the INR has been ≥ 2 for at least two consecutive days or for five days – whichever is the longer.

When do you check INR after starting Coumadin?

Ideally, INR should be checked within 3-5 days of the first warfarin dose. Steady state is achieved in 10-14 days.

Do you hold Coumadin if INR is high?

Minor bleeding, any elevated INR: Hold warfarin, monitor INR, and readjust dose. Consider vitamin K1 (phytonadione) 2.5-5 mg PO once; may repeat if needed after 24 h.

Can you start Coumadin without heparin?

There has been some concern with starting warfarin without concomitant heparin. This is due to the fact that two vitamin K dependent factors, protein C and protein S, are natural anticoagulants and they have very short half-lives of about 7 hours.

How soon can you start Lovenox after surgery?

For medical in-patients enoxaparin should be prescribed at 6pm. For surgical in-patients with a significant reduction in mobility, enoxaparin should be prescribed at 6pm the night before surgery, otherwise it should be started after surgery at the later of: 4 hours post-operatively or 6pm.

Why is warfarin stopped before surgery?

For elective procedures, warfarin should be stopped for 5 to 6 days prior to the procedure to allow gradual normalization of the international normalized ratio (INR).

What does the INRB do?

The Institut National de la Recherche Biomédicale ( INRB) is the national medical research organization of the Democratic Republic of the Congo. The responsible ministry is the Ministry of Scientific Research and Technology. The National Biomedical Research Institute (INRB) was founded in 1984, it is a 70,000 m² establishment.

What is a change in activity required to be reported to IRB?

IRBs are required to function under written procedures. One of these procedural requirements [21 CFR 56.108 (a) (3)] requires ensuring “prompt reporting to the IRB of changes in a research activity.” The completion of the study is a change in activity and should be reported to the IRB.

How many scientists are required to be on the IRB?

21 CFR 56.107 (c) requires at least one member of the IRB to have primary concerns in the scientific area and at least one to have primary concerns in the non-scientific area. Most IRBs include physicians and Ph.D. level physical or biological scientists. Such members satisfy the requirement for at least one scientist.

What information should be included in the IRB roster?

The IRB roster should identify the primary member (s) for whom each alternate member may substitute. To ensure maintaining an appropriate quorum, the alternate’s qualifications should be comparable to the primary member to be replaced. The IRB minutes should document when an alternate member replaces a primary member.
