Stage 1 has a normal x-rays but MRI reveals the dead bone. Stage 2 can be seen on regular x-ray but there is no collapse of the femoral ball. Stage 3 shows signs of collapse (called a crescent sign) on x-ray. Stage 4 has collapse on x-ray and signs of cartilage damage (osteoarthritis).

Osteonecrosis of the hip is a painful condition that occurs when the blood supply to the bone is disrupted. Because bone cells die without a blood supply, osteonecrosis can ultimately lead to destruction of the hip joint and arthritis. Osteonecrosis is also called avascular necrosis of the hip or aseptic necrosis.
How long does core decompression take to heal?
Typically, procedures that involve core decompression—which may or may not also include a bone graft or injection of organic materials—take 6 to 12 months to heal fully. You may or may not need to stay overnight in the hospital.

There is no cure for avascular necrosis, but if it’s diagnosed early using X-rays or MRI, nonsurgical treatments such as activity modification, anti-inflammatory medications, injections, and physical therapy may slow its progression.

Driving is permitted 2-4 weeks after the surgery if you meet the following criteria: ✓ You no longer take narcotic pain medications ✓ You drive an automatic car. If you have a manual car, you could drive after 6 weeks. ✓ You can safely get in and out of your car.
Can you walk after core decompression?
If a bone graft is used in conjunction with the decompression, the recovery period is much longer. Osteotomies generally limit activity for a few months. A total hip replacement often allows patients to begin walking, with support, within days.
Can osteonecrosis be stopped?
If osteonecrosis is diagnosed early enough, collapse and joint replacement can be prevented. To reach these goals, the doctor may use one or more of the following treatments. There is no known pharmaceutical cure for osteonecrosis.
